Why cellulite develops
Cellulite is caused by the structure of female connective tissue: the collagen fibres run vertically, causing fat cells to bulge upwards in a chamber-like pattern. This is exacerbated by reduced lymphatic drainage, poorer blood circulation and the influence of oestrogen — as well as by the decline in collagen density that comes with age.
That is why cellulite is not a question of weight and affects the vast majority of women. It is not found on the surface, but in the tissue beneath — and that is exactly where treatment must begin.
What comes out of it
- Connective tissue — In women, the fibres run vertically, whereas in men they form a network. That is why men almost never have cellulite.
- Hormones — Oestrogen regulates fat distribution and connective tissue. Cellulite typically becomes more pronounced during puberty, pregnancy, the menopause and when taking the contraceptive pill.
- Lymph & Blood Circulation — When fluid accumulates between the fat lobules, the tissue becomes firmer and more uneven.
- Collagen — From around the age of 25, skin density decreases and the skin loses some of its firmness.
What helps against cellulite — and what doesn't
Creams work on the outermost layer of the skin. Cellulite lies deeper — in the connective tissue and the fat beneath it. That is why no cream can permanently smooth out the skin’s texture.
Exercise and a healthy diet are good for the skin and circulation, but they do not alter the structure of the connective tissue. Slim and very athletic women often have even more pronounced cellulite. What really makes a difference works within the tissue itself: on drainage, blood circulation, collagen and the firm connective tissue strands.
Just to clear up any confusion: Cellulite is not a medical condition, but a cosmetic skin condition. Cellulitis is something completely different — a bacterial skin infection characterised by redness, warmth and pain, which requires medical treatment. This page is about cellulite, commonly known as ‘orange peel skin’.

Is cellulite the same as cellulitis?
No, they are two completely different things. Cellulite is the cosmetic ‘orange peel’ skin – a structure of the connective tissue, not a medical condition. Cellulitis, on the other hand, is a bacterial skin infection characterised by redness, warmth and pain, which requires medical treatment, usually with antibiotics. We are concerned exclusively with cellulite.
Will losing weight get rid of my cellulite?
No, cellulite is not a question of weight. It is caused by the vertical structure of female connective tissue and is hormonally influenced by oestrogen — it affects 80 to 98 per cent of women, whether they are slim or not (Bass & Kaminer, Dermatologic Surgery 2020). Very slim and athletic women often have even more pronounced cellulite.
How do Icoone® and Endospheres® work on cellulite?
They work mechanically on the tissue — through lymphatic drainage and improved microcirculation, which are typically impaired in cases of cellulite (Review article, PMC 2023). Icoone® targets the connective tissue with pinpoint accuracy, whilst Endospheres® focuses more on drainage and blood circulation. The aim is to achieve a smoother complexion and firmer tissue — not the permanent removal of cellulite.
Does radiofrequency help with cellulite?
Yes, especially if the skin is also sagging. Monopolar RF heats the deeper layers of the skin, tightens it and stimulates collagen production — studies have shown that this improves the appearance of cellulite, as demonstrated by MRI scans and tissue samples (Goldberg et al., Dermatologic Surgery 2008). At our clinic, this is done using the BTL Exilis® Body.
How many sessions will it take before I see any results?
The change is often noticeable after 2–3 sessions — the tissue feels lighter. The improvement in skin texture is usually visible after 6–8 sessions. We typically work with a 5- or 10-session plan, once or twice a week. The exact number depends on skin quality, the severity of cellulite and lifestyle.
Does cellulite go away — and do the results last?
The appearance of the skin can be significantly improved — and with a little maintenance, these results can be sustained. Because cellulite is rooted in the connective tissue structure, this isn’t about a one-off treatment, but about maintaining the results: Just 1–2 gentle sessions a month are usually enough to maintain drainage and firmness. So you don’t have to start from scratch every spring, but build on what has already been achieved.
Is this treatment suitable for varicose veins or sensitive legs?
In many cases, yes. Endospheres® operates entirely without vacuum, whilst Icoone® uses only a micro-vacuum at the cellular level — both are gentle on the blood vessels, and in cases of venous issues we opt for Endospheres® or adjust the frequency. We do not treat active thrombosis or phlebitis — we clarify this in advance during the consultation.
How much does cellulite treatment cost at alestetics®?
